The relationship between peripheral blood T lymphocyte subpopulations and related cytokines and infection after percutaneous nephrolithotomy in patients with upper urinary tract stones

Abstract:Objective:To investigate relationship between peripheral blood T lymphocyte subpopulations and re-lated cytokines and infection in the patients with upper urinary tract stones after percutaneous nephrolithotomy(PCNL).Methods:A total of 386 patients with upper urinary tract stones who suffered PCNL surgery in our hospi-tal from January 2021 to March 2023 were collected as research subjects.They were grouped into an uninfected group(n=274)and an infected group(n=112)based on whether infection occurred after surgery.The levels of T lympho-cyte subpopulations and related cytokines in the peripheral blood of two groups were compared.Multivariate logistic regression was applied to analyze the influencing factors of postoperative infection in patients with upper urinary tract stones suffered PCNL surgery.Results:There were obvious differences in surgical time,intraoperative bleeding vol-ume,and intraoperative perfusion volume between the infected and uninfected groups(P<0.05).The levels of CD3+[(61.13±6.69)%vs.(63.85±6.74)%],CD4+[(36.82±4.09)%vs.(39.67±4.15)%],CD4+/CD8+[(1.40±0.35)vs.(1.68±0.37)]in the infected group were obviously lower than those of the uninfected group;the level of CD8+[(26.27±2.85)%vs.(23.68±2.82)%],IL-6[(74.72±8.78)ng/L vs.(53.95±8.72)ng/L],TNF-α[(15.84±3.93)ng/L vs.(9.75±3.89)ng/L],and IFN-γ[(3.45±0.87)pg/ml vs.(2.23±0.85)pg/ml]were obvi-ously higher than those of the uninfected group(P<0.05).Multivariate logistic regression analysis showed that surgi-cal time,intraoperative bleeding volume,intraoperative perfusion volume,CD3+,CD4+/CD8+,IL-6,TNF-α,and IFN-γ were all influencing factors for postoperative infection in upper urinary tract stone patients suffered PCNL(P<0.05).Conclusion:The T lymphocyte subpopulations and related cytokines were influencing factors for postopera-tive infection in patients with upper urinary tract stones suffered PCNL surgery.
