The impact of preoperative administration of progesterone on ureteral injuries caused by ureteral access sheaths in ureteroscopy

Abstract:Objective:To study the effect of progesterone administration before operation on ureteral injury caused by ureteral access sheath and perioperative outcome.Methods:A total of 210 patients underwent renal and ureteral calculus treatment in Beijing Daxing District Hospital from October 2021 to August 2022 were selected and assigned to the experimental and control group by random number table method.The experimental group received 20mg intramuscular injection of progesterone 3 days before surgery,while the control group received placebo(0.9%saline 2ml,intramuscular injection).Finally,92 patients in the experimental group and 86 patients in the control group were successfully placed UAS.The rate of ureteral wall injury,the use rate of analgesics and other operation-related indexes were compared between the two groups.Results:26.7%of the patients in the control group had grade 2 or higher ureteral wall damage,and 10.9%of the patients in the experimental group had grade 2 or higher ureteral wall damage.There was a statistical difference in the rate of ureteral wall damage between the two groups(P<0.05).The utilization rate of analgesics in the experimental group was significantly lower than that in the control group[16.30(15/86)vs.30.23(26/92)],and the difference was statistically significant(P<0.05).There was no signifi-cant difference in the overall incidence of complications(P>0.05).Conclusion:Preoperative progesterone therapy can significantly reduce ureteral wall damage caused by the placement of ureteral access sheath during ureteroscope and reduce the use of postoperative painkillers.
