Analysis of the advantages and disadvantages of retroperitoneal laparoscopic ureterolithotomy in the treatment of higher location of upper ureteral calculi

Abstract：Objectives:To compare the surgical efficacy and safety of retroperitoneal laparoscopic ureterolithoto-my(RLU)and ureteroscope lithotripsy(URL)in the treatment of higher location of upper ureteral calculi.Meth-ods:Relevant clinical data of 71 patients with higher location of upper ureteral calculi treated with RLU or URL from January 2018 to December 2022 were retrospectively analyzed.The surgical time,one-time stone removal success rate,one-month stone clearance rate,postoperative fever rate,postoperative abnormal rate of white blood cell count,level of postoperative C-reactive protein,postoperative complications(postoperative ureteral stricture,ureteral perfo-ration,severe hematuria,severe urinary incontinence),hospitalization time,postoperative hospitalization time,first hospitalization expenses,and total hospitalization expenses were compared between the two groups.Results:All 71 surgeries were successful.Compared to the URL group,the RLU group had advantages in one-time stone removal success rate[94.4%(34/36)vs.71.4%(25/35)],one-month stone clearance rate[100%(36/36)vs.80%(28/35)],higher postoperative abnormal rate of white blood cell count[55.6%(20/36)28.6%vs.(10/35)],surgical time[(221.75±83.38)min vs.(45.65±17.35)min],first hospitalization expenses[(15591.27±2714.86)yuan vs.(12897.76±2341.86)yuan],hospitalization time[(14.25±4.92)d vs.(11.08±3.31)d],and postoperative hospital-ization time[(5.77±1.91)d vs.(2.57±0.77)d],and the differences were statistically significant(P＜0.05).There was no statistically significant difference in postoperative fever rate,level of postoperative C-reactive protein,total hospitalization expenses and postoperative complications between the RLU and URL groups(P＞0.05).Conclu-sion:Compared with URL,RLU has certain disadvantages in surgical time and hospitalization time,it more signifi-cant advantages in termes of surgical outcomes,such as one-time stone removal rate and one-month stone clearance rate.RLU is safe and reliable with controlled complications,and there is no significant increase in total hospitalization expenses.
