Risk factor of epididymitis after transurethral resection of prostate for benign prostatic hyperplasia

Abstract:Objective:To explore the risk factors of epididymitis after transurethral resection of prostate(TURP)for benign prostatic hyperplasia(BPH).Methods:Clinical data 826 BPH patients treated in our hospital during January 2015 and December 2021 were retrospectively analysed.The patients were divided into epididymitis and non-epididymitis group.Clinical characteristics of two groups were compared,and the risk factors were analyzed with univariate and multivariate logistic regression analysis.Results:A total of 33 out of 826 patients suffered from epididymitis,with an incidence of 4%.There were significant differences in age,course of disease,hypertension,dia-betes mellitus,preoperative leukocytic positive,prostate volume,residual urine,IPSS score,preoperative prostatic puncture between two groups(P<0.05).Multivariate logistic regression analysis showed that prostate volume[OR= 0.117,95%CI:0.019~0.710;P=0.020],hypertension[OR=7.960,95%CI:1.908~33.198;P=0.004],diabetes mellitus[OR=14.72,95%CI:4.151~52.255;P<0.001]were the independent risk for epididymitis after TURP(P<0.05).Meanwhile,multivariate logistic regression analysis for large volume(>80 ml)BPH showed that prostate volume and operative time were the independent risk for epididymitis after TURP.Conclusion:The incidence of epi-didymitis after TURP for BPH was related to multiple factors.Management should be taken for relieving urethral ob-struction as far as shortening operative time for large volume BPH patients.Strengthening preoperative management for BPH patients with hypertension and diabetes mellitus would be conducive to reduce the risk of epididymitis after TURP.
