Abstract:Objective: To evaluate the significance of fluid therapy in premature infants using a drop of urine for measuring urine
osmolarity (UOC) value with refractometer. Methods: Sixty-two newborn infants were divided into preterm, asphyxiated
preterm and fullterm groups. During 1~5 days of age, their values of UOC and urine sodium (UNa) were measured every
12 hours, and their urine volume (UV) was collected daily. During 2~3 days of age, their values of blood gas and serum os
molarity (SOC) were measured. Results:Their UOC values were measured 584 times totally. (1) The UOC values in pre
term and fullterm groups were 230~300 mmol/L and those in asphyxiated preterm group were higher than 320 mmol/L.
(2) Among the 3 groups, while the UOC values changed, the SOC values remained in a normal range. (3) There was no re
lationship between UOC and UNa. (4) According to the formula: UV=renal solute load/UOC, the value of renal solute loa d
is that UV times UOC, which is dynamic of water intake for infant. Conclusion: Measuring UOC is a simple, reliable, practi
cal and noninvasive monitoring method suitable to guiding infant fluid therapy.
