Abstract:Objective To explore the construction of"non-accompanied wards"and the operation mechanism of hierarchical accompaniment in tertiary public hospitals based on self-care theory. Methods A questionnaire survey was conducted among 231 inpatients who voluntarily participated in the research in the"non-accompanied wards"of Shenzhen Third People's Hospital from May 2024 to November 2024.General information, the acute physiology and chronic health evaluation Ⅱ(second edition)(APACHE Ⅱ)scale, the Barthel index scale, the service demand questionnaire for graded nursing patients, and the questionnaire on the implementation status of accompanying services were used to statistically analyze the needs and current situation of patients and assess the correlation. Results A today of 182 valid questionnaires were retrieved(with an effective recovery rate of 78.79%);The comparison of the scores of care needs among patients with different marital statuses, income levels, care levels,APACHE Ⅱ grades and Barthel index grades showed statistically significant differences(P<0.05).The scores of the overall situation and basic needs, rehabilitation needs, and safety needs of all patients were compared with the actual implementation scores, and the differences were statistically significant(P<0.05).The nursing level and Barthel index grade were negatively correlated with the duration of 24-hour care, while the APACHE II score was positively correlated with the duration of 24-hour care(all P≤0.01).Conclusion When providing companion services, the health status and self-care ability of patients should be combined to rationally allocate companion personnel, and the companion personnel who care for multiple patients simultaneously should be precisely allocated.
