Comparision of the Effects of Transmission Orthodontic Treatment and Traditional Straight Wire Arch Orthodontic Treatment for Skeletal Class Ⅲ Malocclusion in Adolescents During the Permanent Dentition Period

Abstract:Objective To compare the effects of transmission orthodontics and traditional straight wire arch orthodontics in treating skeletal Class Ⅲ malocclusion in adolescents during the permanent dentition period.Methods Sixty adolescent patients with skeletal Class Ⅲ malocclusion who were admitted to Zhengzhou Zhongnuo Stomatological Hospital from January 2022 to May 2023 during the permanent dentition period were selected as the research subjects for a retrospective study.According to the treatment methods,they were divided into a traditional straight wire arch group(27 cases,who received traditional straight wire arch correction)and a transmission straight wire arch group(33 cases,who received transmission orthodontics treatment).Propensity score matching was used to correct the differences between groups.The matching variables included age,gender,severity of bony disease[a point-hasion-b point angle(ANB Angle)],etc.After matching,the baseline characteristics of the two groups of patients were balanced.All patients were continuously observed for 24 months,and the complete follow-up rate was 100%.The therapeutic effects of the two groups of patients,the lip inclination of the maxillary and mandibular central incisors before and after orthodontic treatment[the lower internal angle between the long axis of the maxillary central incisor and the sella-nasion plane(U1-SN angle),the upper internal angle between the long axis of the mandibular central incisor and the mandibular plane(L1-MP angle)],and the satisfaction with the improvement of facial profile were compared.Results The overall clinical efficacy of patients in the transmission straight wire arch group was better than that in the traditional straight wire arch group,and the difference was statistically significant(P<0.05).Twelve months after orthodontic treatment,the U1-SN Angle in the transmission straight wire archway group was smaller than that in the traditional straight wire arch group,and the L1-MP Angle was larger than that in the traditional straight wire arch group.The differences were statistically significant(P<0.05).The total satisfaction rate of patients in the transmission straight wire arch group was higher than that in the traditional straight wire arch group,and the difference was statistically significant(P<0.05).Conclusion Compared with the traditional straight wire arch orthodontic technique,transmission orthodontic treatment can achieve more significant bony improvement effects in intervening in skeletal Class Ⅲ malocclusion in adolescents during the permanent dentition period.At the same time,the compensatory inclination degree of the upper and lower central incisors is smaller,and patients'satisfaction with the orthodontic posterior facial shape is also higher.
