Abstract:Objective To analyze the expression differences of D-dimer(D-D)and four coagulation items in pregnant women of different gestational periods and explore their clinical value. Methods The clinical data of 89 pregnant women in Suiyang District Maternal and Child Health Hospital of Shangqiu City from January 2022 to December 2024 were retrospectively collected. The levels of D-D and four coagulation items[prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(FIB)]of pregnant women in different pregnancy periods were compared.According to the pregnancy outcomes, they were divided into a poor group(9 cases)and a good group(80 cases). The levels of D-D and four coagulation items at different gestational periods of pregnant women with different pregnancy outcomes were compared, and the receiver operating characteristic(ROC)curve was drawn to explore its predictive value. Results The comparison of D-D and FIB levels of pregnant women at different gestational periods, from high to low,was in the labor period, late pregnancy, the mid-pregnancy,and the early pregnancy, and the differences were statistically significant(P<0.05).The comparison of PT,APTT and TT,from long to short, was in the early pregnancy, the mid-pregnancy,the late pregnancy and the period of labor, and the differences were statistically significant(P<0.05).The levels of D-D and FIB in the poor group of pregnant women at each pregnancy were higher than those in the good group,PT,APTT and TT were all shorter than those in the good group, and the differences were statistically significant(P<0.05).The ROC curve shows that D-D,PT,APTT,TT,and FIB at different gestational periods all have good predictive values for poor pregnancy outcomes in pregnant women, and the combined predictive value is even higher. However, there was no statistically significant difference in the combined predictive value among early, middle, and late pregnancy and labor(P>0.05).Conclusion With the increase of pregnancy, the levels of D-D and FIB in pregnant women rise, while PT,APTT,and TT shorten.Moreover,D-D,PT,APTT,TT,and FIB in different pregnancy periods all have good predictive values for poor pregnancy outcomes in pregnant women.
