Analysis of the Influencing Factors of Elevated Serum Progesterone on Trigger Day in Patients with Polycystic Ovary Syndrome Infertility Undergoing in Vitro Fertilization and Embryo Transfer with Long-Acting Long Protocol in the Follicular Phase

Abstract:Objective To explore the influencing factors of elevated serum progesterone(P)levels on the trigger day of the long-acting regimen during the follicular phase in patients with polycystic ovary syndrome(PCOS)and infertility undergoing in vitro fertilization-embryo transfer(IVF-ET).Methods A total of 90 patients with PCOS infertility undergone IVF-ET treatment at Luoyang Central Hospital from January 2023 to October 2024 were selected as the research subjects.All patients were treated with the long-acting long protocol in the follicular phase(triptorelin acetate was injected on the third day of menstruation for down-regulation,and after reaching the target,human menopausal gonadotropin was given for ovulation induction,with regular monitoring and dose adjustment until follicular maturity).Patients were divided into a normal level group and a high level group based on the serum P level on trigger day.The clinical data of the two groups were collected and compared,and the factors with significant differences were included in the multivariate Logistic regression analysis to identify the independent risk factors for elevated serum P on trigger day in PCOS infertility patients undergoing IVF-ET with long-acting long protocol in the follicular phase.Results Among the 90 patients(90 cycles),20 patients(20 cycles)had a serum P level≥1.5 ng·mL-1 on trigger day and were included in the high level group;70 patients(70 cycles)had a serum P level<1.5 ng·mL-1 and were included in the normal level group.The age,total dosage of gonatotropin(Gn),serum estrogen(E2)level,and the proportion of body mass index<24 kg·m-2 in the high level group were all higher than those in the normal level group,and the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that older age,body mass index<24 kg·m-2,higher total Gn dosage,and higher serum E2 level were independent risk factors for elevated serum P on trigger day in PCOS infertility patients undergoing IVF-ET with long-acting long protocol in the follicular phase(OR>1,P<0.05).Conclusion Older age,body mass index<24 kg·m-2,higher total Gn dosage,and higher serum E2 level are risk factors for elevated serum P on trigger day in PCOS infertility patients undergoing IVF-ET with long-acting long protocol in the follicular phase.
