Abstract:Objective To observe the efficacy and safety of programmed death receptor-1(PD-1)inhibitor combined with oxaliplatin+capecitabine(CAPEOX)regimen in neoadjuvant therapy for advanced gastric cancer patients. Methods The clinical data of 80 advanced gastric cancer patients admitted to the First Affiliated Hospital of Guangxi Medical University from January 2023 to December 2024 were retrospectively collected.According to the neoadjuvant treatment regimens, they were divided into a control group(40 cases,CAPEOX regimen)and an observation group(40 cases,PD-1 inhibitor combined with CAPEOX regimen). The clinical efficacy, pathological remission, levels of serum tumor markers carbohydrate antigen(CA)125,CA199,carcinoembryonic antigen(CEA),CA724 before and after treatment, and the occurrence of adverse reactions were compared between the two groups. Results The objective response rate(ORR)and pathological complete response(pCR)rate in the observation group were higher than those in the control group, with statistically significant differences(P<0.05).After treatment, the levels of serum CA125,CA199,CEA,and CA724 in both groups were lower than those before treatment, and the levels in the observation group were lower than those in the control group, with statistically significant differences(P<0.05).There was no statistically significant difference in the degrees of leukopenia, neutropenia, thrombocytopenia, diarrhea, nausea and vomiting, peripheral neurotoxicity, liver and kidney function damage,immune-related pneumonia,hand-foot syndrome, and thyroid function abnormalities between the two groups(P>0.05).Conclusion The neoadjuvant therapy of PD-1 inhibitor combined with CAPEOX for advanced gastric cancer patients is effective, can effectively reduce the levels of serum tumor markers, and does not increase the risk of adverse reactions.
