Abstract:Objective To analyze the effect of butylphthalide(NBP)combined with edaravone in the treatment of first-onset elderly patients with acute ischemic stroke(AIS)and its prognostic influencing factors through a retrospective cohort study. Methods The clinical data of 80 first-onset elderly AIS patients admitted to Pingyu County People's Hospital from July 2023 to November 2024 were retrospectively analyzed. All patients received NBP combined with edaravone treatment. The clinical effect and prognosis of NBP combined with edaravone in the treatment of first-onset elderly AIS patients were statistically analyzed.According to the prognosis, the patients were divided into a poor prognosis group and a good prognosis group. The general data and laboratory indicators of the two groups were compared, and the influencing factors of poor prognosis in first-onset elderly AIS patients were explored through Logistic regression analysis. Results After 14 days of treatment, the total effective rate was 97.50%(78/80).Among them,17 patients had a modified Rankin scale(mRS)score of≥2 and were included in the poor prognosis group, while 63 patients had an mRS score of<2 and were included in the good prognosis group. There were statistically significant differences in age, fibrinogen(FIB),drinking history, and infarction location between the two groups(P<0.05).Logistic regression analysis showed that age, drinking history, infarction location, and FIB were all influencing factors for poor prognosis in first-onset elderly AIS patients(OR>1,P<0.05).Conclusion NBP combined with edaravone has a significant effect in the treatment of first-onset elderly AIS patients, and the prognosis of first-onset elderly AIS patients is affected by age, drinking history, infarction location, and FIB.Clinically, individualized intervention plans can be formulated based on the above factors to reduce the occurrence of poor prognosis in patients.
