Effect of Endoscopic Mucosal Resection under the Guidance of Endoscopic Ultrasound in the Treatment of Rectal Neuroendocrine Tumors with A Diameter Less than 1 cm

Abstract:Objective To explore the efficacy of endoscopic mucosal resection(EMR)under the guidance of endoscopic ultrasound(EUS)in the treatment of rectal neuroendocrine tumors(NETs)with a diameter less than 1 cm.Methods A total of 68 patients with submucosal protrusions diagnosed as rectal NETs with a diameter less than 1 cm by EUS in the Second People's Hospital of Jiaozuo from January 2021 to September 2023 were selected as the research subjects and randomly divided into the endoscopic submucosal dissection(ESD)group and the EMR group,with 34 cases in each group.The surgical-related indicators,lesion resection conditions,complications and recurrence,and serum tumor marker levels were compared between the two groups.Results The operation time and postoperative hospital stay of the EMR group were shorter than those of the ESD group,and the differences were statistically significant(P<0.05);the complete resection rate of the EMR group was higher than that of the ESD group,and the difference was statistically significant(P<0.05);there was no statistically significant difference in the positive rates of vertical and horizontal resection margins between the two groups(P>0.05);there was no statistically significant difference in the total incidence of complications between the two groups(P>0.05);during the 1-year follow-up,no recurrence was observed in either group;one month after the operation,the levels of serum neuron-specific enolase(NSE),chromogranin A(CgA),and vascular endothelial growth factor(VEGF)in the EMR group were lower than those in the ESD group,and the differences were statistically significant(P<0.05).Conclusion Compared with ESD under the guidance of EUS,EMR is more convenient for the treatment of rectal NETs with a diameter less than 1 cm,can more completely remove the lesion,and reduce the levels of serum tumor markers.
