Analysis of Influencing Factors for Poor Postoperative Prognosis in Patients with Acute Anterior Circulation Large Vessel Occlusive Stroke

Abstract:Objective To explore the influencing factors of poor postoperative prognosis in patients with acute anterior circulation large vessel occlusive stroke(ALVOS).Methods A total of 92 patients with ALVOS admitted to Ruzhou First People's Hospital from February 2022 to February 2024 were selected as the research subjects.All patients received mechanical thrombectomy treatment.The modified Rankin scale (mRS)was used to evaluate the prognosis of the patients 3 months after the operation.According to the assessment results,they were divided into a good prognosis group(49 cases)and a poor prognosis group(43 cases).The baseline data of the two groups of patients were compared,and multivariate Logistic regression analysis was used to screen the influencing factors of poor prognosis after mechanical thrombectomy in patients with ALVOS.Results There were statistically significant differences in the previous history of hypertension,history of antiplatelet drug use,occlusive location,National Institutes of Health stroke scale(NIHSS)score at admission,poor collateral circulation,vascular recanalization and severity of cerebral edema between the two groups of patients(P<0.05).The results of multivariate Logistic regression analysis showed that previous history of hypertension,NIHSS score at admission,poor collateral circulation,vascular recanalization,occlusive site and severe cerebral edema were all independent risk factors for poor postoperative prognosis in ALVOS patients.Conclusion The independent risk factors for poor postoperative prognosis in ALVOS patients are previous history of hypertension,NIHSS score at admission,poor collateral circulation,vascular recanalization,occlusal site and severe cerebral edema.Clinically,relevant measures should be taken against the above factors to improve the prognosis of patients.
