Analysis of the Effect of Neuroendoscopic-Assisted Drilling and Drainage in the Treatment of Chronic and Subacute Subdural Hematoma

Abstract:Objective To explore the efficacy of neuroendoscopic-assisted burr hole drainage in the treatment of chronic and subacute subdural hematoma(CSDH).Methods A total of 36 patients with CSDH who underwent surgical treatment at Sanming First Hospital from January to December 2022 were selected as a conventional group(undergoing burr hole drainage),and another 36 patients with CSDH who underwent surgical treatment at Sanming First Hospital from January to December 2023 were selected as an observation group(undergoing neuroendoscopic-assisted burr hole drainage).The surgical-related indicators,hospital stay,recovery status,complication rates,neurological function and daily living abilities,and rates of complete hematoma resolution and recurrence were compared between two patient groups.Results The operation time,postoperative drainage time,and total hospital stay of the observation group were shorter than those of the conventional group,with statistically significant differences(P<0.05).On the 1st,3rd,and 7th days after surgery,the Glasgow coma scale(GCS)scores of the observation group were higher than those of the conventional group,and the total complication rate was lower,with statistically significant differences(P<0.05).At 6 months after surgery,the Markwalder grading scale(MGS)scores of the observation group were lower than those of the conventional group,and the scores of the activities of daily living(ADL)scale were higher than those of the conventional group,with statistically significant differences(P<0.05).The complete hematoma disappearance rates at 3 to 5 days,1 month,and 6 months after surgery in the observation group were higher than those in the conventional group,and the recurrence rate of hematoma on the 6-month follow-up cranial computed tomography(CT)was lower than that in the conventional group,with statistically significant differences(P<0.05).Conclusion Compared with traditional burr hole drainage,neuroendoscopic-assisted burr hole drainage for CSDH shows significant advantages in operation time,postoperative recovery time,postoperative complication rate,neurological function recovery,and hematoma recurrence rate,and has higher safety.
