Analysis of the Effect of Large-Diameter Single-Channel Core Decompression Technique Combined with Local Application of Platelet-Rich Plasma in the Treatment of Avascular Necrosis of the Femoral Head

Abstract:Objective To explore the effect of large-diameter single-hole core decompression technique combined with local application of platelet-rich plasma(PRP)in the treatment of avascular necrosis of the femoral head(ANFH).Methods A total of 80 patients with avascular necrosis of the femoral head admitted to Zhengzhou Hospital of Traditional Chinese Medicine for Orthopedics and Traumatology from January 2020 to December 2022 were selected as the research subjects.They were randomly divided into a control group and an observation group according to the random number table method,with 40 cases in each group.Patients in the control group were treated with small-diameter multi-hole core decompression under hip arthroscopy combined with local application of PRP,while patients in the observation group were treated with large-diameter decompression combined with local application of PRP.The pain degree[visual analogue scale(VAS)score],hip joint function[Harris hip score(HHS)],and magnetic resonance imaging(MRI)effect of the two groups were compared.Results Before the operation,there was no statistically significant difference in VAS score and HHS between the two groups(P>0.05).At 3 months,6 months,12 months,and 18 months after the operation,the VAS scores of both groups were significantly lower than those before the operation,and the VAS scores in the observation group were lower than those in the control group,while the HHS was higher than that before the operation,and the HHS in the observation group was higher than that in the control group,with statistically significant differences(P<0.05).The total excellent and good rate of MRI in the observation group was higher than that in the control group,with statistically significant differences(P<0.05).Conclusion The treatment of avascular necrosis of the femoral head with large-diameter decompression combined with local application of PRP can more effectively improve hip joint function and relieve pain in patients.
