Abstract:Objective To explore the effects of insulin degludec aspart combined with metformin on blood glucose levels and insulin resistance in patients with type 2 diabetes mellitus(T2DM).Methods A total of 74 patients with T2DM admitted to the Third People's Hospital of Nanyang from June 2022 to June 2024 were selected as the research subjects and randomly divided into a metformin group and an insulin group, with 37 cases in each group.Patients in the metformin group were treated with metformin, while those in the insulin group were treated with insulin degludec aspart in addition to metformin. The clinical efficacy, blood glucose indicators, pancreatic function, lipid indicators, antioxidant indicators, and adverse reactions were compared between the two groups. Results The total effective rate of the insulin group(91.89%)was higher than that of the metformin group(72.97%),and the difference was statistically significant(P<0.05).After treatment, the fasting blood glucose(FBG),glycated hemoglobin(HbA1c),and 2-hour postprandial blood glucose(2h PG)levels of the insulin group were lower than those of the metformin group, and the differences were statistically significant(P<0.05).After treatment, the fasting insulin(FINS)level and pancreatic β-cell function index(HOMA-β)of the insulin group were higher than those of the metformin group, and the insulin resistance index(HOMA-IR)was lower than that of the metformin group, and the differences were statistically significant(P<0.05).After treatment, the levels of serum triglyceride(TG),total cholesterol(TC),and low-density lipoprotein cholesterol(LDL-C)of the insulin group were lower than those of the metformin group, and the differences were statistically significant(P<0.05).After treatment, the levels of serum superoxide dismutase(SOD),catalase(CAT),and total antioxidant capacity(T-AOC)of the insulin group were higher than those of the metformin group, and the differences were statistically significant(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination of insulin degludec aspart and metformin in the treatment of T2DM patients has a significant clinical effect, can effectively control blood glucose and lipid levels, improve pancreatic function and antioxidant indicators, and does not increase the risk of adverse reactions.
