Observation on the Effects of Different Administration Times of Porcine Pulmonary Phospholipid Injection on Neonatal Respiratory Distress Syndrome

Abstract:Objective To investigate the effects of different administration timings of porcine pulmonary phospholipid injection in the treatment of neonatal respiratory distress syndrome(NRDS)on the blood gas analysis indicators and mechanical ventilation time of the children.Methods A total of 72 children with NRDS who were hospitalized in Kaifeng Maternal and Child Health Hospital from January 2021 to May 2023 were selected and divided into the early group(administered within 4 hours after birth)and the delayed group(administered within 4 hours before birth and≤8 hours after birth)based on the different administration times of porcine phospholipid injection,with 36 cases in each group.All of them received routine clinical treatment.The therapeutic effects of the two groups of children were compared,and the blood gas analysis indicators[arterial partial pressure of oxygen(PaO2),arterial oxygen saturation(SaO2),arterial partial pressure of carbon dioxide(PaCO2)]were presented.Pulmonary circulation function indicators[pulmonary circulation resistance(PVR),mean pulmonary artery pressure(MPAP),right ventricular stroke index(RVSWI),extravascular lung water content(EVLW)],rehabilitation process[total duration of mechanical ventilation,oxygen therapy and hospital stay]and the occurrence of complications.Results The total effective rate of treatment in the early group of children was 97.22%,which was higher than 77.78%in the delayed group,and the difference was statistically significant(P<0.05).After 72 hours of treatment,the PaO2 and SaO2 of children in the early group were higher than those in the delayed group,and the PaCO2 was lower than that in the delayed group.The differences were statistically significant(P<0.05).After 72 hours of treatment,the levels of PVR,MPAP and EVLW in the early group were lower than those in the delayed group,and the level of RVSWI was higher than that in the delayed group.The differences were statistically significant(P<0.05).After 72 hours of treatment,the total duration of mechanical ventilation,oxygen therapy and hospital stay in the early group was shorter than that in the delayed group,and the difference was statistically significant(P<0.05).The total incidence of complications in the early group was 5.56%.Compared with 13.89%in the delayed group,the difference was not statistically significant(P>0.05).Conclusion Porcine pulmonary phospholipid injection has a more significant therapeutic effect when administered within 4 hours after the birth of children with NRDS.It can effectively improve blood gas analysis indicators and pulmonary circulation function,shorten the total time of oxygen therapy,mechanical ventilation and hospital stay,and has high safety.
