The Effect of Ahmed Glaucoma Valve Implantation in the Treatment of Traumatic Glaucoma and Its Influence on Ocular Hemodynamic Indicators

Abstract：Objective To explore the clinical effect of Ahmed glaucoma valve(AGV)implantation in patients with traumatic glaucoma and its influence on ocular hemodynamic indicators.Methods A total of 70 patients with traumatic glaucoma admitted to Puyang Guangming Hospital from January 2022 to June 2023 were selected as the research objects and divided into a control group and an observation group according to the random number table method,with 35 cases in each group.Patients in the control group received conventional trabeculectomy,while patients in the observation group received AGV implantation.The therapeutic effects,intraocular pressure,best-corrected visual acuity(BCVA),corneal endothelial cell density,hemodynamic indicators[end-diastolic velocity(EDV),peak systolic velocity(PSV),vascular resistance index(RI)]and the occurrence of complications were compared between the two groups of patients.Results The total effective rate of patients in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).At 3,6 and 12 months after the operation,the intraocular pressure of patients in the observation group was lower than that in the control group,and the differences were statistically significant(P<0.05).Six months after the operation,the best corrected visual acuity(BCVA)and corneal endothelial cell density of the patients in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).Six months after the operation,the EDV and PSV of the patients in the observation group were faster than those in the control group,and the RI was greater than that in the control group.The differences were statistically significant(P<0.05).The total incidence of complications in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion AGV implantation for the treatment of traumatic glaucoma can enhance the therapeutic effect,reduce intraocular pressure,increase BCVA,reduce the loss of corneal endothelial cell density,optimize hemodynamic indicators,and has high safety.
