Effect of Esketamine Pretreatment on Intraoperative Circulation,Postoperative Delirium and Stress Response in Children Undergoing Congenital Heart Disease Occlusion Surgery

Abstract:Objective To explore the effects of esketamine pretreatment on intraoperative circulation,postoperative delirium and stress response in children undergoing congenital heart disease occlusion surgery(referred to as"congenital heart occlusion surgery").Methods A total of 80 children scheduled for congenital heart occlusion surgery at Henan Provincial Chest Hospital from January 2022 to March 2024 were selected as the research subjects and randomly divided into a control group and an observation group,with 40 cases in each group.Both groups of children underwent general anesthesia.After anesthesia induction,the observation group was given intravenous injection of esketamine,while the control group was given an equal volume of 0.9%sodium chloride solution intravenously.The heart rate,systolic blood pressure,diastolic blood pressure,cortisol and norepinephrine of the two groups of children were compared at T1(before anesthesia induction),T2(before the start of the operation),T3(15 minutes during the operation),T4(before drug withdrawal),and T5(after awakening).The occurrence of postoperative restlessness and delirium,postoperative recovery time and adverse reactions were also compared between the two groups.Results The heart rate,systolic blood pressure and diastolic blood pressure of the observation group were higher than those of the control group at T2,T3 and T4,and lower at T5,with statistically significant differences(P<0.05);the levels of serum cortisol and norepinephrine of the observation group were lower than those of the control group at T2,T3,T4 and T5,with statistically significant differences(P<0.05);the incidence of postoperative restlessness and delirium in the observation group was 7.50%and 2.50%,respectively,which was lower than 25.00%and 17.50%in the control group,with statistically significant differences(P<0.05);the awakening time,extubation time and anesthesia recovery room stay time of the observation group were shorter than those of the control group,with statistically significant differences(P<0.05);there was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Esketamine pretreatment can effectively maintain the stability of intraoperative circulation function in children undergoing congenital heart occlusion surgery,reduce postoperative delirium and stress response,shorten postoperative recovery time,and has high safety.
