Analysis of the Effectiveness and Risk Factors Affecting the Efficacy of Ultrasound Ablation Therapy for Adenomyosis

Abstract:Objective Exploring the effectiveness and related influencing factors of ultrasound-guided microwave ablation in the treatment of adenomyosis.Methods A total of 108 patients with adenomyosis admitted to Pingmei Shenma Medical Group General Hospital from April 2021 to April 2023 were selected.All patients received ultrasound-guided microwave ablation treatment and were followed up for 3 months.The preoperative and postoperative dysmenorrhea scores,uterine volume,symptoms,quality of life,and clinical efficacy were recorded.According to the recent clinical efficacy at 3 months after surgery,they were divided into a significant efficacy group(very significant clinical efficacy,significant clinical efficacy)and a non significant efficacy group(effective clinical efficacy,ineffective clinical efficacy).Collect clinical related data such as age,body mass index,and pregnancy history of all patients,and use multiple logistic regression analysis to identify factors that affect recent clinical efficacy.Results After surgery,the patient's dysmenorrhea score and symptom severity scale(SSS)score significantly decreased compared to preoperative levels,while the health-related quality of life(HRQL)score significantly increased.The uterine volume significantly decreased compared to preoperative levels,and the differences were statistically significantt(P<0.05);After 3 months of ultrasound-guided microwave ablation treatment for 108 patients with adenomyosis,48 cases(44.44%)showed significant clinical efficacy,29 cases(26.85%)showed significant clinical efficacy,24 cases(22.22%)showed effective clinical efficacy,and 7 cases(6.48%)showed ineffective clinical efficacy.The preoperative dysmenorrhea score,preoperative HRQL score,and ablation ratio of patients in the significantly effective group were compared with those in the non significantly effective group,and the differences were statistically significant(P<0.05);The results of logistic regression analysis showed that the ablation ratio(B=2.258,95%CI:4.640~19.712)was a factor affecting the short-term clinical efficacy of ultrasound-guided microwave ablation treatment in patients(P<0.05).Conclusion Ultrasound guided microwave ablation has significant short-term efficacy in treating patients with adenomyosis.This method has advantages such as minimal trauma and safety,and the ablation ratio is an independent influencing factor on clinical efficacy.
