Analysis of the Clinical and Pathological Characteristics of 6 Cases of Gonadoblastoma

Abstract:Objective To explore the clinical characteristics and pathological diagnosis of gonadoblastoma(GB).Methods Six patients with GB admitted to the First Affiliated Hospital of Zhengzhou University from 2019 to 2023 were selected as the research subjects.Specialized examinations,imaging tests and laboratory tests were conducted on them.Their clinical manifestations,results of specialized examinations,imaging tests and laboratory tests,immunohistochemical characteristics,microscopic examination conditions and pathological diagnosis results were analyzed.Results All 6 patients had gonadal hypoplasia clinically;The levels of follicle-stimulating hormone(FSH)and luteinizing hormone(LH)in the peripheral blood of 4 patients all increased to varying degrees.Meanwhile,the levels of testosterone(T)and estradiol(E2)generally decreased.The level of β-human chorionic gonadotropin(β-HCG)in 1 patient significantly increased.The karyotypes of 4 patients were 46,XY,and that of 1 patient was 45,X(24)/46,XY(76).Chromosomal chimeric karyotype GB tumor cells are composed of primordial germ cells and immature sex cord cells.Among them,one patient is accompanied by asexual cell tumor,and one patient is accompanied by asexual cell tumor and choriocarcinoma.Immunohistochemical analysis revealed that germ cell octamer-binding transcription factor 4(OCT4),stem cell factor receptor(CD117),placental alkaline phosphatase(PLAP),and zinc finger transcription factor 4(SALL4)were positive.Sex cord cells are positive for α-inhibin,calcioretinal protein(CR),and β-HCG.Conclusion GB is a rare gonadal tumor,and its clinical manifestations and pathological features have certain specificity.
