Comparison of the Application Value of three Doses of Letrozole in Obese Polycystic Ovary Syndrome Infertility Patients

Abstract:Objective To compare and analyze the application value of three doses of letrozole in obese patients with polycystic ovary syndrome(PCOS)infertility.Methods A retrospective analysis was conducted on the clinical data of 108 obese PCOS infertility patients admitted to Zhengzhou Maternal and Child Health Hospital,from October 2022 to May 2023.They were divided into high-dose group,medium dose group,and low-dose group according to different treatment.Methods with 36 cases in each group.The low-dose group received oral administration of 2.5 mg of letrozole on the 2nd to 5th day of menstruation for 5 consecutive days;The medium dose group received oral administration of 5 mg of letrozole on the 2nd to 5th day of menstruation for 5 consecutive days;The high-dose group received oral administration of 7.5 mg of letrozole on the 2nd to 5th day of menstruation for 5 consecutive days.The number of dominant follicles,endometrial thickness,ovulation rate,ovulation induction time,estradiol(E2),luteinizing hormone(LH),incidence of adverse reactions,pregnancy rate,and miscarriage rate among three groups of patients were compared.Results The number of dominant follicles in the high-dose group was higher than that in the low-dose and medium dose groups,and the thickness of the endometrium was lower than that in the low-dose and medium dose groups,with statistical significance(P<0.05);The ovulation rate of the high-dose group was higher than that of the low-dose and medium dose groups,and the ovulation induction time was shorter than that of the low-dose and medium dose groups,with statistical significance(P<0.05);The high-dose group had higher levels of human chorionic gonadotropin(HCG)day E2 compared to the low-dose and medium dose groups,and lower levels of LH compared to the low-dose and medium dose groups,with statistically significant differences(P<0.05);There was no statistically significant difference in the incidence of adverse reactions among the three groups(P>0.05);There was no statistically significant difference in the abortion rate among the three groups(P>0.05);The pregnancy rate of the high-dose group was higher than that of the low-dose and medium dose groups,and the difference was statistically significant(P<0.05).Conclusion When using different doses of letrozole to treat obese PCOS,high doses have better effects compared to low and medium doses.They can further improve ovarian function,promote ovulation,and increase pregnancy rates without increasing the incidence of adverse reactions.Clinically,the optimal treatment dose can be selected based on the actual situation of the patient.
