Comparison of Clinical Characteristics and Pregnancy Outcomes between Iatrogenic Preterm Birth and Spontaneous Preterm Birth in Pregnant Women

Abstract:Objective To compare the clinical characteristics and pregnancy outcomes between iatrogenic preterm birth and spontaneous preterm birth in pregnant women and to provide a reference for the timing of medical intervention in high-risk preterm pregnancies.Methods A retrospective analysis was conducted on the clinical data of 110 pregnant women with normal preterm births from October 2022 to October 2023 in the Department of Obstetrics at the Yellow River Sanmenxia Hospital.The cases were divided into an iatrogenic group(47 cases)and a spontaneous group(63 cases)based on whether the preterm birth was due to active termination of pregnancy.Information on general conditions and health status during pregnancy were collected,and the modes of delivery and the health status of mothers and infants were compared between the two groups.Results The iatrogenic group had significantly higher proportions of women with a history of abortion,preterm birth,and cesarean section,as well as higher gestational age and cesarean section rates compared to the spontaneous group.The incidence rates of preeclampsia,surgical diseases during pregnancy,fetal distress,oligohydramnios,placenta previa,and antepartum hemorrhage were higher in the iatrogenic group than in the spontaneous group.The incidence rates of multiple pregnancies,premature rupture of membranes,abnormal fetal position,unexplained preterm birth,vaginal delivery rate,and pneumonia were lower in the iatrogenic group than in the spontaneous group,with statistically significant differences(P<0.05).The incidence rates of postpartum complications in the iatrogenic and spontaneous groups were 6.38%and 11.11%,respectively,with no statistically significant difference(P>0.05).Conclusion There are significant differences in clinical characteristics between iatrogenic and spontaneous preterm births in pregnant women,which are closely related to the condition of preterm infants.Identifying the causes of preterm birth and related factors of health status,and providing standardized guidance and intervention,can help reduce the incidence of preterm birth and improve the health status of preterm infants.
