The Value of"Home-community-hospital"Combined Care Model Based on Network Platform in Integrated Management of Type 2 Diabetes Mellitus

Abstract：Objective To explore the value of"home-community-hospital"combined care model based on network platform in integrated management of type 2 diabetes mellitus(T2DM).Methods A total of 105 T2DM patients who were admitted to Quanzhou First Hospital Affiliated to Fujian Medical University from January 2021 to June 2023 were selected and divided into an observation group(53 cases)and a control group(52 cases)using random number table method.The control group took routine care,and the observation group adopted"home-community-hospital"care model based on network platform.The comparisons were performed on the blood glucose,blood pressure,lipid level,self-management behavior,quality of life and patients'satisfaction in both groups.Results The observation group achieved lower levels of glycosylated hemoglobin,fasting blood glucose,systolic blood pressure,diastolic blood pressure,total cholesterol(TC),triglyceride(TG)than the control group after intervention(P<0.05).Compared with the control group,the summary of diabetes self-care activities(SDSCA)score and nursing satisfaction in the observation group after intervention were higher,and the diabetes specific quality of life(DSQL)in the observation group was lower(P<0.05).Conclusion"Home-community-hospital"care model based on network platform in the integrated management of T2DM is conductive to decreasing the levels of blood glucose,blood pressure and lipid,improving self-management behavior and quality of life,and increasing patients'nursing satisfaction.
