Study on the Efficacy and Safety of Endoscopic Thyroidectomy Via Trans-thoracic Approach for the Treatment of T1 Papillary Thyroid Carcinoma

Abstract：Objective To explore the efficacy and safety of endoscopic radical thyroidectomy via the trans-thoracic approach for the treatment of T1 papillary thyroid carcinoma.Methods A retrospective study was conducted on 80 patients with T1 papillary thyroid carcinoma who underwent surgery at Guangzhou Women and Children's Medical Center Affiliated with Guangzhou Medical University from August 2023 to May 2024.The patients were divided into an endoscopic group and an open group according to the surgical approach,with 40 cases in each group.The endoscopic group underwent surgery via the trans-thoracic approach,while the open group received traditional open surgery with an anterior cervical incision.The surgical indicators,and incidence of postoperative complications were compared between the two groups.Results The operation time in the endoscopic group was significantly longer than that in the open group,with a statistically significant difference(P<0.05).The volume of drainage on the first postoperative day and the total drainage volume were higher in the endoscopic group than in the open group,with statistically significant differences(P<0.05).The duration of drainage tube placement was longer in the endoscopic group than in the open group,with a statistically significant difference(P<0.05).The total hospitalization cost was also significantly higher in the endoscopic group than in the open surgery group,with a statistically significant difference(P<0.05).There were no statistically significant differences in intraoperative blood loss,the number of central lymph node dissections,and the number of lymph node metastases between the two groups(P>0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Endoscopic thyroidectomy via the trans-thoracic approach for the treatment of T1 papillary thyroid carcinoma is clinically satisfactory,safe,and reliable.It has certain advantages for patients with high aesthetic requirements.
