Effect of Health Management Platform Model Based on Internet Technology on Discharged Patients with Cardiovascular Disease

Abstract：Objective To analyze the effect of health management platform model based on internet technology on discharged patients with cardiovascular disease.Methods 60 patients with cardiovascular 2021 who were discharged from the First Affiliated Hospital of Nanchang University from January to January 2022 were selected and analyzed.In order to draw lots to maintain the ratio of 1:1 of the number of people were divided into a control group,an observation Group,30 cases.The control group received routine follow-up,and the observation Group received follow-up and management based on internet health management platform.The mastery of health knowledge,self-management ability,reexamination rate and quality of life were compared between the two groups.Results The scores of mastery of health knowledge,self-management ability and quality of life in the observation group were higher than those in the control group,the differences were statistically significant(P＜0.05).Conclusion The application of health management platform mode based on Internet technology can improve the management effect and efficiency of patients with cardiovascular diseases,and it is beneficial to improve their health knowledge reserve and self-management ability,and ensure their timely reexamination,and contribute to the improvement of their quality of life.
