Exploration to mechanism of reduced cardiac function in rheumatic diseases based on BTLA, Treg

Abstract:Objective To explore the mechanism of reduced cardiac function based on the B , T lymphocyte attenuator (BTLA), regulatory T cells (Treg) in patients with rheumatism. Methods 345 patients with rheumatic diseases from First Affiliated Hospital of Anhui University of Chinese Medicine, during July, 2009 August, 2013, including rheumatoid arthritis (RA, 153 cases), ankylosing spondylitis (AS, 76 cases), Sjogren′s syndrome (SS, 56 cases) and osteoarthritis (OA, 60 cases). Heart function was detected by echocardiography (UCG), expression of Treg and BTLA were detected by flow cytometry . Results ( 1 ) Cardiac dysfunction rate was 38 . 8% in 345 patients with rheumatism, cardiac dysfunction rates were 49.6%, 35.5%, 30.3%, 23.3% in RA, AS, SS, OA respectively. (2) Correlation analysis showed that heart function parameters such as E peak, E/A, EF% were positively correlated with BTLA, E peak, E/A, FS%were positively correlated with Treg, E/A were negatively correlated with RF, A peak were positively correlated with C3, C4, IgG, ESR, EF%, FS%were negatively correlated with ESR (P < 0.05 or P < 0.01). (3) Compared with control group, the expression of BTLA, Treg in peripheral blood decreased in rheumatism patient (P < 0.01). Cardiac function parameters such as E peak, E/A, EF%, FS% significantly decreased, A peak significantly increased (P < 0.05 or P < 0.01). (4) Compared with BTLA normal group, cardiac function parameters such as E peak, E/A, FS%significantly decreased while A peak significantly increased in BTLA abnormal group. Compared with Treg normal group, E peak, E/A, EF%, FS%significantly decreased in Treg abnormal group ( P < 0 . 05 or P < 0 . 01 ) . Conclusions Disorders of negative co-stimulatin signal inhibiting BTLA-HVEM , Treg immune tolerance may be one of the reasons of heart function reduction of rheumatic patients.
