A Case Report of Aquagenic Syringeal Acrokeratoderma

Abstract:A 6-year-old female presented with the appearance of white papules and oedematous plaques on both hands after contact with water for 15 days.The patient had a longstanding history of a similar condition for years,and skin lesions could gradually be resolved upon discontinuation of water contact.Physical examinations showed thickened keratinisation of the skin on the dorsal surfaces of both hands and the lateral margins of both wrists.Following a 5-minute water exposure,white papules and edematous plaques developed on these areas,characterized by soft texture,absence of tenderness,and diminished sensation,which were remarkable compared with pre-contact of water source.Lesions resolved and normalized within 20 minutes post-water exposure cessation.The water immersion test was positive.Histopathological examination of the lesions showed hyperkeratosis of the epidermis,thickening of the stratum spinosum and dilatation of the ducts of the sweat glands.Finally,the patient was diagnosed with aquagenic syringeal acrokeratoderma.After a two-month course of topical treatment with Desonide Cream in conjunction with Boric Acid and Zinc Oxide Ointments,the patient's skin lesions showed significant improvement.
