Artificial Dermatitis Misdiagnosed as Pemphigus:a Case Report

Abstract:This study reported a case of artificial dermatitis misdiagnosed as pemphigus.A 78-year-old male presented with erythema and erosion with itching on the trunk and limbs for 3 months.He was misdiagnosed with pemphigus,and poorly responded to oral glucocorticoids and topical antibiotics.Routine pathological examination of skin lesions at the edge of erythema on the chest revealed skin ulcer formation,scab formation on the surface,granulation tissue proliferation in the superficial dermis,and no obvious special features.Immunopathology showed no immune complex deposition.Autoimmune blister antibodies were negative.Combined with the patient's history of chronic renal insufficiency and solitary personality,he was finally diagnosed with artificial dermatitis.After treatment with Niaoduqing granules and Jinshuibao capsules for kidney protection,and gabapentin against nerve sensitization,the patient had relieved pruritus and stopped scratching.Erosion area of chest and back was reduced,and erythema color was relieved.At the 3 months of post-discharge follow-up,the erosions and ulcerations had primarily re-epithelialized,with residual hyperpigmentation observed.
