Clinical observation on the efficacy of radiotherapy after keloid excision

Abstract:Objective To evaluate the efficacy of radiotherapy following keloid resection.Methods A total of 70 patients(75 lesions)who underwent keloid resection in the Department of Burn and Plastic Surgery(45 cases)and Department of Dermatology(25 cases)at Dongguan Tungwah Hospital between May 2020 and June 2022 were enrolled.For 34 keloids in low-recurrence areas,electron beam radiotherapy was administered at a total dose of 15 Gy(5 Gy per fraction,once daily for 3 consecutive days).For 41 keloids in high-recurrence areas,the total dose was 20 Gy(5 Gy per fraction,once daily for 4 consecutive days).Clinical efficacy was assessed at 3,6,and 12 months post-radiotherapy,with grading of radiation dermatitis evaluated at 3 months.During 12-24 months of follow-up,the effective symptom control duration,recurrence rate,and complication incidence were compared between low and high-recurrence areas.Results At 3,6,and 12 months post-radiotherapy,no significant differences were observed in total effective rates between low and high-recurrence areas(88.24%vs 87.80%,91.18%vs 90.24%,97.06%vs 92.68%)or in radiation dermatitis grading at 3 months(all P>0.05).During follow-up,no significant differences were found in effective symptom control duration,recurrence rate,or complication incidence between the two areas(all P>0.05).Conclusion Postoperative radiotherapy with large-fraction,low-frequency regimens(different doses for low/high-recurrence areas)demonstrates favorable efficacy for keloids,with low recurrence and complication risks,supporting its clinical application.
