A case of psoriasis complicated with pemphigus erythematosus

Abstract:A 48-year-old male patient,presented with generalized scaly patches for 8 years and widespread blisters for the last month.Dermatological examination revealed diffuse skin lesions characterized by vesicles on an erythematous basis,with flaccid blisters prone to rupture(Nikolsky sign positive).Ruptured blisters were covered by yellowish scabs,accompanied by small amount of pale-yellow clear exudate.Skin biopsy showed the skin lesions at the blister site of the left elbow presented hyperkeratosis of the epidermis,focal acantholysis in the spinous layer,blister formation at the basal layer,superficial dermal neovascularization.The scaly patches on the left upper arm showed parakeratosis of the epidermis,microabscesses in the epithelium,club-shaped downward extension of trochanterellus,mild superficial dermal inflammatory cell infiltration.Direct immunofluorescence demonstrated reticular deposition of IgG(++)and C3(+)between spinous layer cells.Serological testing confirmed positive anti-desmoglein 1(Dsg1)and anti-desmoglein 3(Dsg3)antibodies.Combining clinical manifestations,histopathological findings,and immunological results,the diagnosis was established as psoriasis complicated with pemphigus erythematosus.Treatment with glucocorticoids combined with acitretin capsules led to complete resolution of skin lesions.During a 1.5-year follow-up,no recurrence was observed.
