A case of bilateral asymmetric herpes zoster with human immunodef iciency virus infection

Abstract:A 50-year-old male patient presented with the chief complaint of blisters accompanied by pain on the left side of the chest,the left side of the neck and shoulder,as well as the right side of the chest and back for 3 days.Dermatological examination revealed clustered blisters on an erythematous base distributed along the cutaneous nerves on the left chest,the left neck and shoulder,and the right chest and back.Some of the blisters were broken and crusted.Auxiliary examination showed a positive result in the detection of human immunodeficiency virus(HIV)antibody test.The patient was eventually diagnosed as HIV infection complicated with bilateral asymmetric herpes zoster.Systemic treatment with famciclovir,mecobalamin,and pregabalin was administered,and penciclovir cream was applied topically.One week after the treatment,the blisters began to dry up and the pain was alleviated.Two weeks later,all the blisters were completely dried up and crusted,and the pain was significantly reduced.One month later,the skin lesions subsided,the scabs fell off,and there was almost no pain.
