Clinicopathological analysis of 4 cases of sebaceous carcinoma

Abstract:Objective To analyze the clinicopathological features,diagnosis,and differential diagnosis of sebaceous carcinoma(SC).Methods Clinical data of 4 SC patients were collected to examine histopathological characteristics and immunophenotypic features,with a review of relevant literature.Results The patients(2 males,2 females)aged 52-87 years included 2 periocular and 2 extraocular cases.One case was associated with Muir-Torre syndrome,presenting as a subcutaneous cyst-like lesion,while the remaining 3 showed papillomatous skin lesions.Microscopically,3 cases exhibited lobular growth composed of basaloid and sebaceous cells;1 case showed poorly differentiated features with necrosis,keratinization,extensive stromal invasion,and Bowenoid changes in the surface epithelium.All 4 cases displayed significant cytological atypia with 9-21 mitotic figures per 10 high-power fields.Immunohistochemistry revealed positivity for CK,CK7,P63,EMA,and AR,and negativity for CK20,Ber-EP4,S100,Syn,CGA,and GCDFP-15.The Ki67 proliferation index ranged from 30%to 50%.Follow-up data(3-41 months)for 3 cases showed no recurrence or metastasis.Conclusion SC is a rare and aggressive tumor that requires differentiation from primary skin tumors such as basal cell carcinoma,squamous cell carcinoma,and Merkel cell carcinoma.Sebaceous cells serve as critical diagnostic clues,and immunohistochemistry can improve diagnostic accuracy for suspected cases.
