Clinical and histopathological analysis of 22 cases with vulvar fibroepithelial stromal polyps

Abstract:Objective To summarize the clinical manifestations,and features of histopathological and immunohistochemistry of vulvar fibroepithelial stromal polyps(FEPs).Methods Clinical and pathological data of 22 female patients with FEPs admitted to the Department of Dermatology,Beijing Chaoyang Hospital,Capital Medical University,between 2015 and 2023 were retrospectively analyzed.Their clinical manifestations,histopathological characteristics,treatment,and prognosis were systematically summarized.Results All 22 patients were female,with solitary lesions confined to the vulvar region.None reported subjective symptoms.Histopathology revealed polypoid lesions with approximately normal epidermis and loose dermal tissue containing fibrovascular stroma.Some blood vessels showed thickened walls.The stroma contained abundant cellular components,predominantly spindle-shaped and stellate cells,with scattered or rare multinucleated giant cells.Nuclear pleomorphism was observed in some cells.Immunohistochemistry and special stains revealed stromal cells were positive for Factor XIIIa,partially positive for CD34,and positive for estrogen receptor(ER)and progesterone receptor(PR),but negative for smooth muscle actin(SMA).Alcian blue staining of the stroma was negative.All patients underwent complete surgical excision,and no recurrence was observed during follow-up.Conclusion FEPs are rarely reported in China.Histopathology and immunohistochemistry play critical roles in the diagnosis and differential diagnosis of FEPs.Complete surgical excision yields favorable outcomes,while incomplete resection may lead to recurrence.Long-term follow-up is recommended for all patients.
