A case of Nevus of Ota associated with central nervous system malignant melanoma and cerebral

Abstract:A 30-year-old female patient presented with a chief complaint of"brownish rashes around the right eye for over 30 years and sudden onset of headache for 1 day".Dermatological examination showed bluish-black maculopapules located 2 cm above the right periorbital region and the superciliary arch,and bluish-black patches on the oral mucosa.Neurosurgical examination showed obvious neck stiffness,and both Brudzinski's sign and Kernig's sign were positive.Magnetic resonance imaging(MRI)indicated a high probability of meningioma accompanied by hemorrhage,and the possibility of metastatic tumor could not be ruled out.Additionally,a small amount of subarachnoid hemorrhage was detected.Histopathological examination(HE)revealed a melanin-rich tumor.The tumor cells had abundant cytoplasm,uniform nuclear size,prominent nucleoli,and approximately 1 mitotic figure per square millimeter.Immunohistochemical results showed positive expressions of HMB45(+++),Melan-A(+++),P16(++),and SOX-10(++),absence of BAP1,and a Ki-67 index of 1%-3%.The fluorescence in situ hybridization(FISH)test for genes related to malignant melanoma was positive.The final diagnosis was Nevus of Ota,malignant melanoma,and cerebral hemorrhage.On November 24,2023,a craniotomy was performed under general anesthesia for the resection of multiple tumors in the right temporo-occipital lobe.Postoperatively,symptomatic treatments including fluid replacement,reduction of intracranial pressure,nerve nourishment,vascular support,and hormone therapy were administered.The patient is currently still under follow-up.
