Efficacy of platelet-rich fibrin therapy:injection combined with wound coverage for chronic refractory wounds

Abstract:Objective To analyze the efficacy of platelet-rich fibrin(PRF)for chronic refractory wounds through combination therapy of injection and wound coverage.Methods A total of 120 patients with chronic refractory wounds admitted to the Department of Emergency Surgery of Ningde Hospital Affiliated to Ningde Normal University from January 2023 to June 2024 were selected.According to the random number table method,they were divided into the control group(n=60)and the observation group(n=60).Venous blood of the patients was drawn to prepare covering-type PRF and injection-type PRF for standby.The control group was treated with PRF wound coverage,while the observation group received a dual pathway treatment combining PRF injection with PRF wound coverage.The positive rates of wound bacterial cultures before treatment,at 14 days and 28 days after treatment,the relative mRNA expression levels of wound tissue growth factors[platelet-derived growth factor(PDGF),vascular endothelial growth factor(VEGF),fibroblast growth factor(FGF),transforming growth factor(TGF),insulin-like growth factor(IGF)],and inflammatory factors[interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α)]were compared between the two groups.The complete wound healing rates at 14 and 28 days after treatment were also compared.Additionally,the complete wound healing time and the scores of the Vancouver Scar Scale(VSS)after complete wound healing were compared between the two groups.Results At 14 days and 28 days after treatment,the positive rates of wound bacterial cultures,and the relative expression levels of IL-1β and TNF-α mRNA in wound tissues were both decreased(P<0.05),and those in the observation group were lower than those in the control group at the same points(all P<0.05).The relative expression levels of PDGF,VEGF,FGF,TGF and IGF mRNA in wound tissues were all increased,and those in the observation group were higher than those in the control group(all P<0.05).Conclusion The combination therapy of PRF injection and PRF wound coverage can significantly shorten the complete wound healing time of patients with chronic refractory wounds,inhibit wound inflammation,promote the secretion of wound tissue growth factors,and increase the wound healing rate.
