Analysis of clinical features and therapeutic efficacy of rosacea in 107 Demodex-positive childhood

Abstract:Objective To retrospectively analyze the clinical features and therapeutic efficacy of in Demodex-positive childhood rosacea across different age groups.Methods A total of 107 pediatric patients(age≤12 years)diagnosed with Demodex-positive rosacea at 940 Hospital of PLA Joint Logistic Support Force,between January 2013 and December 2023 were enrolled.Patients were divided into preschool age group(n=20,<7 years)and school age group(n=87,7-12 years).The treatment protocol included topical metronidazole gel twice daily for 4-8 weeks;non-responders after 8 weeks switched to oral ornidazole(20 mg/kg twice daily,maximum single dose≤500 mg)for 4 weeks,while responders continued topical metronidazole for an additional 4 weeks.Finally,the clinical data of pediatric patientsc were further retrospectively analyzed,including demographic data,clinical symptom scores,therapeutic efficacy.Results Among 107 patients(42 males,65 females),the median age was 10.0(7.0,11.0)years,with papulopustules being the most common clinical manifestation(65.42%).School-age patients had higher symptom scores than preschool-age patients.In the preschool group,topical metronidazole achieved response rates of 80.00%at week 4,95.00%at week 8,and 100%after switching to oral ornidazole at week 12.In the school-age group,topical metronidazole alone showed response rates of 48.28%at week 4 and 55.17%at week 8,which increased to 95.40%after oral ornidazole at week 12.The highest cure rates in both groups were observed between weeks 8 and 12.Conclusion Topical metronidazole alone is highly effective in preschool-age children with Demodex-positive rosacea.Efficacy of topical metronidazole decreases with age,but early systemic ornidazole can significantly improve therapeutic efficacy in non-responding pediatric patients.
