Abstract:A 26-year-old female presented with scattered yellowish-brown hyperpigmented macules on the left hemifacial region for over 20 years.Dermatological examination revealed multiple irregularly shaped pigmented lesions of varying sizes, partially coalescing into patches. These lesions exhibited unilateral distribution along the left facial midline with sharply demarcated borders, demonstrating striking asymmetry compared to the unaffected right side.Confocal laser scanning microscopy examination showed a notable increase in pigmentation in the basal layer of the affected skin.Histopathological examination indicateed that changes in the epidermis were minimal, but proliferating melanocytes were visible in the basal layer, with a focal,freckle-like distribution. There was mild infiltration of lymphocytes around superficial dermal blood vessels.Based on the clinical and examination findings, the patient was finally diagnosed as partial unilateral lentiginosis. The patient declined laser intervention and was instead treated with vitamin C and Shugan Huoxue Granule. The patient is currently being followed up.
