A retrospective study of the relationship between pemphigus vulgaris and thyroid dysfunction

Abstract:Objective To investigate the relationship between pemphigus vulgaris(PV)and thyroid diseases.Methods 107 patients diagnosed with PV,and 250 healthy individuals were included in this retrospective study.Serum samples were collected from these groups to determine and compare the expression levels of triiodothyronine(T3),thyroxine(T4),free triiodothyronine(FT3),free thyroxine(FT4),thyroid stimulatinghormone(TSH),anti-thyroglobulin antibody(TGAb)and anti-thyroid peroxidase antibody(TPO-Ab).The prevalence of thyroid diseases and the expression level of thyroid indicators were compared according to age,sex and severity of illness.Results 1.Compared to the control group,the PV group exhibited significantly lower levels of T3,T4,FT3,FT3 and TSH(P<0.05).2.Compared with the control group,the incidence of subclinical hyperthyroidism,low T3 syndrome and autoimmune thyroiditis in PV group was higher than that in control group,and the difference was statistically significant(P<0.05).3.Patients with severe PV exhibited lower levels of T3,FT3 and TSH compared to those with moderate PV(P<0.05).4.In PV patients,with the increase of the severity of the disease,the levels of dsgl and dsg3 increased,however,these levels were not associated with the prevalence of thyroid diseases or the expression level of thyroid autoantibodies.Conclusions The possibility of autoimmune thyroid disease(AITD)occurring in PV patients with positive thyroid autoantibodies is high.It is therefore necessary to examine thyroid function and thyroid autoantibodies in patients diagnosed with pemphigus during the initial diagnosis and follow-up.These examinations can detect the existence of thyroid diseases in their early stages thus enabling simultaneous treatmentof the two diseases.
