Cutaneous mycosis fungoides presenting as pigmented purpuric dermatosis:a case report

Abstract:A 68-year-old male patient with progressive aggravation of ecchymosis on extremities and sacrococcygeal region for 4 years.Dermatological examination revealed diffuse or scattered ecchymosis and brown patches on both lower extremities and the sacrococcygeal region.The lesions on both lower extremities were mainly located in the anterior tibial region.The ecchymosis was fused into patches,and normal skin could be seen between the lesions.Histopathological examination of skin lesions on the lateral thigh showed that the spine layer was thickened,some skin processes were decreased and widened,and focal sponges were formed.Lymphoid cells infiltrating around the vessels in the superficial dermis,red blood cells overflow and a small amount of hemosiderin deposition could be seen,and lymphocytes with large nuclear concentration were observed migrating into the epidermis.Immunohistochemistry results demonstrated positivity for CD3,CD4,CD8,and CD45RO,and negativity for CD20 and CD79a,indicating a T-cell phenotype for the lymphocytes invading the epidermis.TCR gene rearrangement showed monoclonal positive.Diagnosis:mycosis fungoides.The patient was treated with recombinant human interferon α2b injection at a dose of 300 IU via intramuscular injection every other day,along with narrow-spectrum UVB irradiation treatment three times a week,no changes in skin lesions were observed after treatment.
