An analysis of misdiagnosis and mistreatment of a case of dermatofibrosarcoma protuberans with the appearance of crater like nodule
Abstract:A 16-year-old male patient presented with a painless reddish tumour on right forearm, which slowly increased for more than 10 years. One year before the visit, a "crater"-like depression emerged in the center of the tumour, and a small amount of dark brown scabs without ulceration were seen at the bottom of the tumour. The patient was diagnosed as keratoacanthoma, then CO2laser removal combined with photodynamic therapy was given to the patient, and histopathologic examination was performed at the same time, which indicated typical dermatofibrosarcoma protuberans(DFSP) changes, but the result wasn't confirmed by immunohistochemical examination, and the patient was treated with ALA-PDT continuously. Due to the poor curative effect, immunohistochemical examination was made up, and the staining for CD34 was positive. Combined with clinical manifestations and pathological findings, the patient was finally diagnosed as dermatofibrosarcoma protuberans. The process of misdiagnosis and mistreatment of the case suggests that some clinicians may have low awareness of DFSP, especially juvenile DFSP and may be not fully aware of the importance of histopathological examination.
