Application of negative pressure wound therapy combined with free flap transplantation in the treatment of patients with extensive burns

Abstract:Objective To observe the application effect of negative pressure wound therapy(NPWT)combined with free flap transplantation(FFT)in the treatment of patients with extensive burn surface(EBS).Methods From February 2022 to August 2024,A prospective clinical controlled trial was conducted in 115 patients with EBS treated in Department.of Burn and Plastic Surgery,The First People's Hospital of Shangqiu.After generating the random sequence by computer grouping method,the patients were divided into the con-ventional group(57 cases)and the combined group(58 cases).The conventional group was treated with FFT,while the combined group was treated with Ayong NPWT combined with FFT.The patients were followed up for 6 mouths after treatment.The inflammatory indexes,wound healing,rehabilitation process and complications of the two groups were compared.Results After treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and IL-1β in the combined group were lower than those in the conventional group,and the level of interleukin-10(IL-10)in the combined group was higher than that in the conventional group(t=6.040,3.548,3.233,3.596;P<0.05).The levels of vascular endothelial growth factor(VEGF),platelet-derived growth factor(PDGF),basic fibroblast growth factor(bFGF)and epidermal growth factor(EGF)in the combined group were higher than those in the conventional group(t=3.395,2.693,3.518,3.415;P<0.05).The flap vascularization time,wound complete closure time,drainage tube removal time,passive activity time and hospital stay in the combined group were lower than those in the conventional group(t=3.713,3.422,5.862,7.585,2.616;P<0.05).By the end of follow-up,the incidence of complications in the combined group was 8.62%(5/58),which was lower than 29.82%(17/57)in the conventional group(x2=8.355;P<0.05).Conclusion NPWT combined with FFT can reduce the inflammatory reaction and promote wound healing in patients with EBS,which has a positive negative effect on speeding up the rehabilitation process and reducing the risk of complications.
