Clinical effects of medial sural artery perforator flap combined with stellate popliteal scar relief on improving childhood popliteal scar contracture deformity

Abstract：Objective To investigate the clinical effects of medial sural artery perforator flap combined with stellate popliteal scar relief on improving childhood popliteal scar contracture deformity.Methods Total 8 cases of popliteal scar contracture deformity,treated by medial sural artery perforator flap combined with stellate popliteal scars relief from Department of Burns and Plastic Surgery,Affiliated Hospital of Zunyi Medical University,June 2017 to December 2023 were retrospectively analyzed.All the patients suffered postburn scar,and limited knee extension or claudication was found with different degrees,and scar ulcers occurred in 3 of them.The wound area after scar contracture resection and release was from 5.0 cm × 3.0 cm to 11.0 cm × 4.5 cm,and the flap area was from 6.0 cm × 3.0 cm to 12.0 cm × 5.0 cm.All the donor sites were directly sutured.The flap survival,the donor site healing,and knee joint mobility were recorded.Results All the flaps survived,and the flap pedicles were transferred by skin incision.All were performed by open-track metastasis.All the patients were followed up and ranged from 3 to 34 months,with an average of 8.5 months.The flap color,texture and appearance were excellent.There was no obvious scar hyperplasia in the donor site.The knee joint was actively and passively straightened to 0 degrees,and squat freely was found.Patients family members are satisfied with the operation effects.Conclusion The medial sural artery perforator flap combined with stellate popliteal scar relief is one of the effective surgical methods for improving childhood popliteal scar contracture deformity.
