Clinical effect of the application of rhomboid flap to the sacrococcygeal pilonidal sinus

Abstract:Objective To explore the clinical effect of the application of local rhomboid flap to the sacrococcygeal pilonidal sinus.Methods The retrospective analysis was performed on the clinical data of 35 patients with sacrococcygeal pilonidal sinuses admitted to the Department of Burn Plastic Surgery and Wound Repair in the Second Hospital of Lanzhou University from June 2013 to August 2023.Intraop-eratively,the surgical incision line was marked with methylene blue,and sufficient amount of methylene blue was injected into the skin surface opening of the pilonidal sinus for staining tracing to ensure that the lesion was completely resected.According to the extent of pilonidal sinus lesion and surrounding soft tissue infection,30 cases of secondary wounds were repaired after complete resection of the pilonidal sinus.One-stage complete excision of the lesion and thorough debridement,with the wound treated using vacuum sealing drainage(VSD),followed by a second-stage repair using rhomboid flaps to address secondary wounds in 5 cases.All patients were followed up for 3 months to 10 years to assess the healing of the surgical incision in the sacrococcygeal region,the survival of the rhomboid flap and the recurrence of the pilonidal si-nus.Results All patients' sacrococcygeal rhomboid flaps survived well without significant fullness.The flaps donor and recipient sites incisions healed well,leaving behind superficial linear scars.None of the patients experienced recurrence after surgery,indicating satis-factory surgical results.Conclusion The local rhomboid flap is simple to harvest and has a definite clinical effect in repairing secondary wounds after sacrococcygeal pilonidal sinus excision,making it worthy of clinical application.
