Bimaxillary protrusion:a retrospective study of anterior maxillary osteotomy combined with subapical mandibular osteotomy

Abstract:Objective To explore the safety and efficacy of maxillary anterior segmental osteotomy combined with mandibular subapical osteotomy in treating bimaxillary protrusion.Methods A retrospective analysis was performed on the clinical data of 30 patients who underwent maxillary anterior osteotomy plus mandibular apical osteotomy for correcting maxillary protrusion deformity at the Depart-ment of Plastic Surgery,North Campus of Shanghai First People's Hospital Affiliated to Shanghai Jiao Tong University from 2021 to 2023.Preoperative and postoperative 6-month lateral cephalograms were analyzed,with measurements taken for 13 bone tissue parameters and 10 soft tissue parameters.Statistical analyses were conducted on the measured data,and patient satisfaction was assessed via question-naires.Results All the 30 patients had primary healing after operation.The occlusal relationship was normal,the appearance of gummy smile and beaked deformity was significantly improved,and they were satisfied with the surgical effect.Conclusion Under the surgery-first ap-proach,the combination of anterior maxillary osteotomy and mandibular subapical osteotomy,selectively combined with genioplasty,can ef-fectively correct adult bimaxillary protrusion and significantly improve the contour of parrot beak deformity in appropriately selected patients.
