Efficacy and safety study of CO2 fractional laser and Er:YAG fractional laser combined with topical application of com-pound zinc undecylenate triamcinolone acetonide ointment in the treatment of hyperplastic scarring

Abstract:Objective To investigate efficacy and safety of CO2 fractional laser and Er:YAG fractional laser combined with com-pound zinc undecylenate triamcinolone acetonide ointment for treatment of hyperplastic scarring with topical Laser-Assisted Drug Delivery(LADD).Methods Retrospectively 93 patients with hyperplastic scarring admitted and diagnosed in our hospital from January 2021 to January 2023 were selected for the study,and were classified as observation group(n=48)and control group(n=45)according to the treat-ment plan.CO2 fractional laser combined with glucocorticoid for LADD treatment was given to the observation group,and Er:YAG fractional laser combined with glucocorticoid for LADD treatment was given to the control group.Patients in both groups were treated three times with an interval of 2 months.The efficacy,vancouver scar scale(VSS),visual analogue scale(VAS),dermatology life quality index(DLQI),scar serum indexes[Transforming Growth Factor-β1(TGF-β1),Matrixmetalloproteinase-2(MMP-2),typeⅠ/Ⅲcollagen ratio],wound healing time after the first treatment,incidence and duration of adverse effects were collected and analyzed in both groups.Results The total ef-fective rate(44/48,91.67%)of observation group was higher than that(34/45,75.56%)of control group(P=0.035);VSS,VAS and DLQI scores of observation group were lower than those of control group(P<0.05).TGF-β1,MMP-2,and typeⅠ/Ⅲcollagen ratio of the observation group were lower than those of the control group(P<0.05).There was no statistically significant difference between observation group's wound healing time after the first treatment,incidence and duration of adverse reactions and control group(P>0.05).Conclusion CO2 fractional laser combined with compound zinc undecylenate triamcinolone acetonide ointment by LADD for the treatment of patients with hyperplastic scars showed better improvement in terms of efficacy,quality of life,modulation of TGF-β1,MMP-2,and typeⅠ/Ⅲcollagen ratio.
