Treatment of epicanthic fold with the modified Uchida procedure combined with blepharoplasty in correcting upper eyelid scar deformity

Abstract:Objective To investigate the clinical effects of the modified Uchida procedure in the treatment of epicanthic fold combined with blepharoplasty in correcting upper eyelid scar deformity and restoring a natural upper eyelid appearance.Methods From March 2021 to January 2024,8 patients on 12 sides with upper eyelid scar deformity were treated by treatment of epicanthic fold with the modified Uchida procedure combined with blepharoplasty in the Department of Burn and Plastic Surgery of the First Affiliated Hospital of Guangxi Medical University.During the operation,the epicanthic fold was performed using Uchida procedure flap to form a natural inner canthal angle.The scar tissue on the eyelid was removed and released by blepharoplasty,and the levator palpebrae superioris muscle was adjusted to obtain a natural and aesthetic upper eyelid appearance.Results In this group of 8 patients,6 to 18 months of follow-up was obtained.The swelling was mild,recovery was fast,wounds healed in one stage.The inner canthal angle and the eyelid fold were natural with-out scarring pulling,improvement from pre—operative condition obvious with a high satisfaction rate.One patient had conjunctival edema,two had superficial ecchymosis which gradually subsided within 2 weeks post-operatively.One patient had rounding of the inner canthal angle repaired again after 3 months.No other patients had complications such as infection or hematoma at surgical site.Conclusion It is a good method to use the modified Uchida procedure combined with blepharoplasty to remove scar tissue in the eyelid while fully releasing skin tension,allowing patients with post-traumatic eyelid injuries to regain a natural,comfortable,and aesthetically pleasing eyelid appearance.
