Research progress on the combined use of negative pressure wound therapy and topical oxygen therapy in the treatment of chronic refractory wounds

Abstract:The management of chronic refractory wounds(CRW)remains a longstanding challenge in the field of wound healing.Negative pressure wound therapy(NPWT),although established as a primary intervention for CRW,induces a hypoxic microenvironment that impairs angiogenesis,granulation tissue formation,and re-epithelialization during prolonged application,thereby limiting its efficacy in ischemic-hypoxic wounds.Topical oxygen therapy(TOT),administered via continuous diffusion or pressurized systems to deliver localized high-concentration oxygen,demonstrates potential in ameliorating tissue hypoxia by enhancing oxygen supply to wound beds.Consequent-ly,the combined application of NPWT and TOT may address the limitations of isolated NPWT-induced hypoxia through synergistic thera-peutic effects.Nevertheless,current clinical evidence supporting this combined modality lacks broad validation,with critical uncertainties persisting in optimal therapeutic parameters and underlying mechanisms.This review systematically examines the clinical applications and research on the combined use of NPWT and TOT for CRW,aiming to establish a theoretical foundation for its clinical implementation and widespread adoption.
