Effect of PRP combined with VSD on refractory deep second-degree burn wound and its effect on macrophage polarization

Abstract:Objective To investigate the efficacy of platelet rich plasma(PRP)combined with VSD in the treatment of refractory deep second-degree burn wounds and the effect on macrophage polarization.Methods A total of 108 patients with refractory deep Ⅱ degree burn wounds treated in our hospital from January 2020 to June 2022 were collected as research objects.According to the treatment method,all enrolled patients were divided into the simple VSD group and PRP+VSD group with 54 cases each.The control group was treated with VSD alone after debridement,and PRP+VSD group was treated with PRP+VSD after debridement.The differences in wound healing,scar hyperplasia after wound healing and macrophage polarization were compared between the two groups.Results The wound healing rate of the PRP+VSD group was higher than that of the simple VSD group at 7 d,14 d and 21 d,respectively,and the difference was statistically significant(P<0.05).At 21 days after surgery,there was a statistically significant difference in efficacy grading between the simple VSD group and the PRP+VSD group(P<0.05).The total effective rate of the PRP+VSD group was higher than that of the VSD group,and the difference was statistically significant(P<0.05).The wound healing time in the PRP+VSD group was shorter than that in the VSD group,and the Vancouver Scar Scale(VSS)score was lower than that in the VSD group,with statistical significance(P<0.05).iNOS positive cell count in the PRP+VSD group was lower than that in VSD group and CD206 positive cell count was higher than that in the VSD group at 7 d,14 d and 21 d after treatment,the difference was statistically significant(P<0.05).Conclusion PRP combined with the VSD can significantly promote the healing of refractory deep second-degree burn wounds,and the specific mechanism involves the regulation of macrophage polarization.
