Repair of hand wounds with free lower abdominal perforator flap

Abstract:Objective To explore the clinical effect of free lower abdomen perforator flap in the repair of skin and soft tissue defects of the hand.Methods From March 2010 to December 2013,twenty-eight patients with skin and soft tissue defects of the hand were involved in the stndy.Their wounds were repaired with lower abdominal perforator flaps with abdominal wall shallow artery (20 cases) and superficialiliaccircumflexarlery (8 cases) as the pedicle.The flap area was from 8.0 cm × 4.0cmtol6.0cm × 12.0cm.Results All flaps survived in stage one.One patient with infection postoperative healed after bacterial culture and multiple dressing changes with sensitive antibiotics.Twelve patients received skin flap thinning surgery at the second intention.Scar contracture occurred in three patients and finger extension and flexion activity was limited.The fingers' mobility improved after scar modification surgery.All patients were followed up from 5 to 24 months.The flaps were soft,the donor site of abdominal flap was healed well,there was no obvious discomfort,and only linear scar remained.Conclusion The free lower abdominal perforator flap was convenient for obtaining a large area.The wound could be covered in one application.The donor area was hidden and could be sutured directly.This surgical method is ideal method for repairing skin and soft tissue defects of the hand.
